Bring your photo ID (passport works), any prior vaccine records you have, and any medical records related to chronic conditions you take medication for. Form completion
The civil surgeon completes Form I-693 with all required exam findings, lab results, and vaccine records.
Sealing
The completed Form I-693 is sealed in the USCIS envelope, signed and dated. The envelope cannot be opened or it voids the form.
Second visit - pickup
You return to pick up the sealed envelope. We provide an itemized receipt if you need it for your attorney.
